Concepto y definición de EPOC
La EPOC se caracteriza por la presencia de obstrucción crónica y
poco reversible al flujo aéreo asociada a una reacción inflamatoria
anómala principalmente frente al humo del tabaco1,2.
La EPOC se asocia a inflamación crónica con remodelado que
afecta a las vías aéreas, parénquima y arterias pulmonares1,2.
La EPOC es una enfermedad prevenible,
tratable y con repercusión sistémica1,2.

Definición
La “Enfermedad Pulmonar Obstructiva Crónica” (EPOC) se caracteriza por la presencia de obstrucción crónica y poco reversible al flujo aéreo asociada a una reacción inflamatoria anómala principalmente frente al humo de tabaco, aunque sólo una cuarta parte de los fumadores desarrolla EPOC. La exposición continuada a productos de la combustión de biomasa en ambientes cerrados también se ha asociado a EPOC. El déficit homozigoto de alfa-1-antitripsina se asocia a enfisema precoz en fumadores.La EPOC es una enfermedad prevenible, tratable y con repercusión sistémica.

FENOTIPOS: HETEROGENEIDAD GENÉTICA DE LA EPOC
La EPOC es una enfermedad heterogénea que engloba multiples
subfenotipos.
Los diferentes fenotipos pueden tener distinta contribución genética por lo que los diferentes estudios
deben considerar esta heterogeneidad

The reference values are shown as dotted lines [FEV1 = 140mL (panel a), and FEV1 AUC1-4h = 220mL (panel b)]. Data are least squares means with 95% CI.

Efectos sobre la TOS
17– 20% de los pacientes experimentan tos esporádica a los 15 segundos tras la inhalation de indacaterol– Es una tos muy corta, de 5 seg. (10 seg. en fumadores) – Más frecuente en género femenino y en fumadores
Solo el 6.8% de los pacientes refieren tos como efecto adverso en los ensayos clínicos
Generalmente está bien tolerada.
Esta TOS no hace discontinuar el tratamiento en los ensayos clínicos

Conclusiones de Indacaterol en ensayos clinicos Indacaterol es un broncodilatador eficaz de 24 h. Efecto de 24 h de
duración, con mejorías significativas en el FEV1 comparado con placebo, tiotropio y formoterol
La eficacia del Indacaterol se mantiene durante las 52 semanas
Indacaterol es efectivo para el control de sintomas :– reduce el porcentaje de días de mal control de los EPOC– reduce de forma significativa la disnea comparado con placebo y
con importante mejoría con respecto a formoterol y tiotropio – reduce la necesidad de medicación de rescate comparado con
formoterol y tiotropio
Indacaterol mejora de forma significativa la calidad de vida y reduce las exacerbaciones , comparado con placebo
Indacaterol es muy seguro y está bien tolerado, sin efectos sobre el potasio, la glucosa o eventos cardiacos
CONCLUSIONES La EPOC es una patología crónica y muy prevalente.
Donde se detectan diferentes fenotipos La medicación indicada desde el inicio son los
broncodilatadores, solos o asociados. En estos momentos disponemos del Indacaterol que
es un broncodilatador de muy rápida acción , el único que es de larga duración y asegura un efecto las 24 h (mejor cumplimiento al hacer una única dosis diaria).
Seguridad : no ha demostrado efectos adversos;es el broncodilatador muy seguro.
