METOD ( I )

c a s e s w i t h c r y p t o r c h i s m and n e v e r i n c a s e s w i t h i n t r a s c r o t a l a b
n o r m a l i t i e s .

The m a t e r i a l c o n s i s t s o f 123 p a t i e n t s 3 - 75 y e a r s o l d . T h i r t e e n
p a t i e n t s were i n v e s t i g a t e d b i l a t e r a l l y , t h e o t h e r s u n i l a t e r a l l y .
Two p a t i e n t s were i n v e s t i g a t e d t w i c e .
The i n v e s t i g a t i o n b e g a n w i t h an a b d o m i n a l a o r t o g r a p h y i n o r d e r t o
d e t e r m i n e t h e p o i n t a t w h ic h t h e t e s t i c u l a r a r t e r y a r i s e s , u s u a l l y
f r o m t h e v e n t r a l w a l l o f t h e a o r t a b e t w e e n t h e r e n a l an d t h e i n f e -

r i o r m e s e n t e r i c a r t e r i e s . The s e l e c t i v e c a t h e t e r mu s t h a v e t h e
S - f o r m shown i n F i g , 2, I , and i t i s i m p o r t a n t t h a t t h e d i s t a l
b e n d h a s t h e r i g h t l e n g t h . The s e l e c t i v e c a t h e t e r g e t s c a u g h t i n
t h e t e s t i c u l a r a r t e r y whi ch i s e a s y t o i d e n t i f y b e c a u s e o f i t s
c h a r a c t e r i s t i c c o u r s e . I n a b o u t 10 °fo o f c a s e s t h e t e s t i c u l a r
a r t e r y s p r i n g s f ro m t h e r e n a l a r t e r y and s e l e c t i v e i n v e s t i g a t i o n
i s n o t p o s s i b l e .
The s e l e c t i v e i n j e c t i o n o f c o n t r a s t medium was c a r r i e d o u t
m a n u a l l y a t m o d e r a t e p r e s s u r e . 1 - 2 ml were i n j e c t e d i n c a s e s
o f a g e n e s i s o r c r y p t o r c h i s m and 3 ml when t h e t e s t i s was i n t h e
s c r o t u m . The f i l m s wer e e x p o s e d u s i n g a f i l m c h a n g e r and i n c a s e s
w i t h s c r o t a l a b n o r m a l i t i e s a m a g n i f i c a t i o n a n g i o g r a p h y was p e r f o r
med w i t h a m i c r o f o c u s t u b e . The t e s t i s i n t h e s c r o t u m was e x p o s e d
f r e e f ro m t h e b od y so t h a t t h e g on ad d o s e became a s low as 0 , 3 -
0 , 5 mGy p e r e x p o s u r e .
I n 29 c a s e s t h e s e l e c t i v e i n v e s t i g a t i o n c o u l d n o t be p e r f o r m e d .
I n 10 c a s e s t h e t e s t i c u l a r a r t e r y o r i g i n a t e d f rom t h e r e n a l a r t e r y
and i n 19 c a s e s t h e a r t e r y c o u l d n o t be s e l e c t i v e l y i n v e s t i g a t e d
u s u a l l y b e c a u s e o f a t h e r o s c l e r o s i s i n e l d e r l y men. 109 t e s t i c u l a r
a r t e r i e s were s e l e c t i v e l y i n v e s t i g a t e d . Only a f ew c o m p l i c a t i o n s
w ere n o t e d .
The d i a m e t e r o f t h e t e s t i c u l a r a r t e r y was m e a s u r e d w i t h a v e r n i e r
c a l l i p e r . I n men t h e d i a m e t e r i s n o r m a l l y 1 , 2 6 Ì 0 , 6 0 mm. I n c a s e s
w i t h a g e n e s i s and c r y p t o r c h i s m t h e a r t e r y i s s l e n d e r and i n c a s e s
w i t h a c u t e e p i d i d y m i t i s o r l a r g e t u m o r s t h e d i a m e t e r i s g r e a t e r .
A l o t o f s l e n d e r b r a n c h e s a r i s e s f rom t h e t e s t i c u l a r a r t e r y t o t h e
s u r r o u n d i n g s . Some o f t h e s e a r e c o m m u n i c a t i o n s t o t h e r e n a l o r lum

U s u a l l y o n e , o r s o m e t i m e s tw o , c o m p a r a t i v e l y l a r g e b r a n c h e s r u n
t o t h e r e t r o p e r i t o n e a l s p a c e c a u d a l t o t h e k i d n e y an d t o t h e f a t t y
c a p s u l e o f t h e k i d n e y .
I n t h e m a t e r i a l 35 n o n - p a l p a b l e t e s t e s were s e l e c t e d f o r i n v e s t i
g a t i o n by means o f t e s t i c u l a r a n g i o g r a p h y . I n 6 c a s e s t h e i n v e s t i
g a t i o n was n o t s u c c e s s f u l . Among t h e o t h e r s a c r y p t o r c h i d t e s t i s was
f o u n d i n 15 c a s e s an d t e s t i c u l a r a g e n e s i s i n 14* Twelve o f t h e p a -

t e s t i s . When s e l e c t i v e l y i n j e c t e d t h e c o n t r a s t medium i s s e e n i n
t h e t e s t i s w i t h i n a f ew s e c o n d s and i n t h e v e i n s 1 5 - 2 0 s e c o n d s
l a t e r .
I n t h e whole m a t e r i a l 81 t e s t i c l e s were s e l e c t e d f o r m a g n i f i c a t i o n
a n g i o g r a p h y . I n 65 c a s e s t h e i n v e s t i g a t i o n c o u l d be c a r r i e d o u t .
The r e s u l t was c o n s i d e r e d e x c e l l e n t i n 53 c a s e s , s a t i s f a c t o r y i n
9 a nd p o o r i n 3 c a s e s . The f i n a l d i a g n o s i s was o f t e n b a s e d on t h e
o p e r a t i v e f i n d i n g s and a l s o , i n 60 i o f t h e c a s e s , on m i c r o s c o p y o f
b i o p s y m a t e r i a l o r t h e e n t i r e t e s t i s . I n 13 c a s e s i t was p o s s i b l e ,
i n t h e l i g h t o f t h e a n g i o g r a p h i c f i n d i n g s , t o p o s t p o n e e x p l o r a t i o n
o f t h e t e s t i s . V a r i o u s p a t h o l o g i c a l c o n d i t i o n s were i n v e s t i g a t e d .
I n h y d r o c e l e t h e c o l l e c t i o n o f f l u i d l i e s b e t w e e n t h e two l a y e r s
o f t h e t u n i c a v a g i n a l i s . The v e s s e l s o f t h e p a r i e t a l l a y e r become
d i s p l a c e d f ro m t h e t e s t i s . No o t h e r c h a n g e s h a v e b e e n o b s e r v e d .
No c h a n g e s h a v e b e e n d e m o n s t r a t e d i n s m a l l s p e r m a t o c e l e s .
I n hem atomas i n t h e s c r o t u m o n l y an i n c r e a s e d d i s t a n c e b e t w e e n
t e s t i s and t h e s c r o t a l w a l l i s s e e n , p r o v i d e d t h a t t h e hematoma i s
s i t u a t e d i n t h e cavum s c r o t i when i t m im ics a h y d r o c e l e . Hematomas
i n t h e f u n i c e l may p r o d u c e v e n o u s s t a s i s . R u p t u r e o f t h e t e s t i c u l a r
c a p s u l e c a n be s e e n and may i n d i c a t e t h e n e e d f o r an o p e r a t i o n .
I n e p i d i d y m i t i s t h e e p i d i d y m i s i s t h i c k e n e d , t h e p a r e n c h y m a f i l l s

v a s c u l a r i z e d and when s m a l l c o u l d o n l y be s e e n b e c a u s e t h e i n t r a -
t e s t i c u l a r a r t e r i e s a r e m i s s i n g i n t h e l o c a l i t y o f t h e tu m o r . The
c a r c i n o m a s a r e r i c h l y v a s c u l a r i z e d and t h e r e f o r e more e a s i l y r e
c o g n i z a b l e .
V a r i c o c e l e i s a l s o e a s y t o r e c o g n i z e ow ing t o t h e w id e n e d v e i n s .
Some r e t r o p e r i t o n e a l l e s i o n s h a v e t h e t e s t i c u l a r a r t e r y a s a
f e e d i n g a r t e r y . T h i s was t h e c a s e i n one r e n a l c a r c i n o m a and two
u r e t e r i c c a r c i n o m a s .
I t i s p o s s i b l e t o p e r f o r m t e s t i c u l a r a n g i o g r a p h y and i t i s some
t i m e s o f g r e a t i m p o r t a n c e f o r t h e f u r t h e r t r e a t m e n t o f t h e p a t i e n t .
