• Ceftolozane exhibits linear, dose dependent pharmacokinetic ( Cmax
and AUC).
• The binding of ceftolozane and tazobactam to human plasma proteins
is low
• Ceftolozane is eliminated in the urine. The mean T1/2 for ceftolozane
and tazobactam is approximately 3 hours and 1 hour, respectively
• For ceftolozane, the time that the plasma concentration exceeds the
minimum inhibitory concentration of ceftolozane for the infecting
organism is the best predictor of efficacy
• For tazobactam, the pharmacodynamic index associated with efficacy
was determined to be the percentage of the dose interval during which
the plasma concentration of tazobactam exceeds a threshold value

Indications
Ceftolozane/tazobactam is indicated for the treatment of the following
infections in adults:
• Complicated intra-abdominal infections
• Acute pyelonephritis
• Complicated urinary tract infections
Efficacy has been demonstrated in clinical studies against the pathogens
listed under each indication that were susceptible to ZERBAXA® in vitro:
IAI = intra-abdominal infection; UTI = urinary tract infection.

– Valorado y aceptado en lista de trasplante pulmonar

• Mujer 67a
• Sin alergias conocidas ni hábitos tóxicos
• HTA en tratamiento con IECAs
• DM tipo 2 en tratamiento con Metformina
• Episodio depresivo mayor con ideación suicida en 2011. Tratamiento con Olanzapina, Venlafaxina y Lorazepam.
• Fractura L1 tras caída accidental; fractura osteoporótica, recibiendo tratamiento con bifosfonato ev.
• Trasplante unipulmonar izdo en Julio/16 por fibrosis pulmonar idiopática diagnosticada en 2012. Rechazo persistente A3. Inmunosupresión con Micofenolato 500 mg c/24h, Tacrolimus 6.5 mg c/12 h y Urbason 8 mg c/24h.
• Dolor paranasal dcho. En la exploración del 8/01/17 mostraba ligeros signos de inflamación, con borramiento de surco nasogeniano dcho y leve enrojecimiento. Durante las 24h seguientes: progresión de la lesión + febrícula Amoxicilina-Clavulánico a 2 g c/8h ev.
• TC Facial + Órbita (10/01/17): CELULITIS PERIORBITARIA – Afectación inflamatoria de tejidos blandos periorbitario
superficiales en órbita derecha – Engrosamiento en region nasal inferior y mayor lateralizacion
hacia region nasogeniana derecha que aparece engrosada e hipodensa por edema de mucosa pero sin colecciones organizadas.
– No se visualiza contraste en la vena angular derecha, confluente de la vena facial.
– Importante componente inflamatorio en las celdillas etmoidales y senos maxilar y etmoidal.

• El 11/01/17 presenta franco empeoramiento del estado general, obnubilación y evidente progresión de la lesión facial, con aparición de rinorrea purulenta bilateral.
• Se avisa a UCI, activándose Código Sepsis, cambiando cobertura antibiótica a Meropenem + Linezolid + Amfotericina B y realizándose nuevo TC cráneo-facial.
• Ingresa en UCI en Shock séptico: VM + drogas vasoactivas.
Caso clínico 2 CELULITIS PERIORBITARIA
El TC facial y cervical realizado muestra: • Clara progresión en 24h con aumento de volumen de la lesión
hipodensa situada en vestíbulo nasal derecho con captación periférica de sus limites, contenido hipodenso homogéneo y extensión hacia región nasal donde la lesión muestra márgenes irregulares y contenido aéreo, indicando colección probablemente abierta a cavidad nasal con captación intensa de contraste en relación a cornetes y tabique nasal que aparecen preservados, con desviación del tabique hacia la izquierda.
• Moderada progresión de los cambios inflamatorios hacia región nasogeniana y orbitaria claramente situada en región preseptal, sin lesión intraorbitaria.
• Ocupación discontinua de celdas etmoidales y engrosamiento de mucosa periférico en senos maxilares, esfenoidal y frontal con captación de contraste de mucosa en relación a cambios inflamatorios que también afectan a planos grasos de región hemifacial y parotidea derecha.
• Persiste la compresión de vena angular en relación a celulitis preseptal orbitaria derecha ,sin otras anomalías vasculares.

• Tratamiento empírica con Meropenem + amikacina + linezolid + amfotericina B IV por sospecha de mucor.
• Se cursaron cultivos locales (secreciones nasales, cultivo de biopsia) y hemocultivos.
• PL negativa
• Control foco: Valoración ORL de debridamiento de la zona pero no lo consideraron necesario.
• A las 24 hs: Tratamiento dirigido tras identificarse P. aeruginosa XDR (únicamente sensible a amikacina y colistina), inició Ceftolozano/tazobactam a dosis de 3 g/ 8 hs IV + Amikacina, se retira al 3 día al comprobar S a C/T.

• TAC craneal (23/01): disminución significativa de las imágenes a nivel sinusal y nasal, aunque no desaparición completa.
• Se decidió en sesión multidisciplinaria mantener el tratamiento por 3 semanas con Ceftolozano/tazobactam (13/01 al 9/02).

• Requirió soporte vasoactivo con dosis máxima de
Noradrenalina de 0.15 mcg/kg/min. No ha presentado deterioro de la función renal.
• IOT + VM al ingreso, FIO2 0.45 máximo. Fue extubada el día 20/01, sin incidencias. Se da de alta con GN 3 lpm , manteniendo SpO2 96%.
• Durante los primeros 6 días de ingreso, se suspendió la inmunosupresión por niveles tóxicos de tacrolimus (>60 mcg/ml), y leucopenia. Reintroducción con buena tolerancia.
• Dos determinaciones de CMV negativas. • Alta a planta de Pneumología el 25/01/17.

• 58-year-old man with HIV admitted to the intensive care unit for a CAP due to influenza A virus with high fever and suffering a severe acute respiratory distress syndrome.
• In the second week he presented a ventilator-associated tracheobronchitis and sacral pressure ulcer with necrotic tissue: multiresistant P. aeruginosa only susceptible to colistin and amikacin.
• 3 days later: Bacteriemia by multiresistant P. aeruginosa .
• Succesful treatment with C/T 1,5g/8h, 10 days.
