Interest in this approach was revived after several studies identified patient subgroups with 20%–40% rates of locoregional recurrence after mastectomy and chemotherapy.
These subgroups, which included women with four or more positive lymph nodes or an advanced primary tumor (a tumor of 5 cm or a tumor invading the skin or adjacent musculature), were thought most likely to benefit from a course of postmastectomyradiotherapy.
