• LOS 5ARI REDUCEN EL RIESGO A LARGO PLAZO DE RAO Y NECESIDAD DE UN TRATAMIENTO QUIRÚRGICO DEL ADENOMA
• 5ARI :UNA REDUCCIÓN RELATIVA DEL DIAGNÓSTICO DE CÁNCER DE PRÓSTATA DE 23 A 25% A EXPENSAS DE UNA MENOR INCIDENCIA DE CÁNCER PROSTÁTICO DE BAJO GRADO, ESTE RESULTADO SE ASOCIÓ A UN INCREMENTO ABSOLUTO EN LA INCIDENCIA DE CÁNCERES DE PRÓSTATA DE ALTO GRADO (0,7% CON FINASTERIDE Y 0,5% CON DUTASTERIDE).
EFECTOS ADVERSOS
1. FUNCIÓN SEXUAL E INCLUYEN DISMINUCIÓN O PÉRDIDA DE LA LIBIDO, DISFUNCIÓN ERÉCTIL Y TRASTORNOS DE LA EYACULACIÓN(EYACULACIÓN RETRÓGRADA O DISMINUCIÓN DEL VOLUMEN DEL SEMEN)
2. FINASTERIDE : 5,9% MÁS HOMBRES CON DISFUNCIÓN ERÉCTIL EN COMPARACIÓN CON EL GRUPO PLACEBO, 5,5% MÁS EN DISMINUCIÓN O PÉRDIDA DE LIBIDO Y 13% MÁS EN DISMINUCIÓN DEL VOLUMEN EYACULATORIO.
3. DUTASTERIDE: DERECTIL: 3,3% MÁS HOMBRES EN COMPARACIÓN CON EL GRUPO PLACEBO, DISMINUCIÓN O PÉRDIDA DE LIBIDO EN 2,3% MÁS Y DISMINUCIÓN DEL VOLUMEN EYACULATORIO EN 1,2% .
4. GINECOMASTIA :1% MÁS DE LOS PACIENTES CON DUTASTERIDE.

Sexual dysfunction is not always drug related. Assessment of sexual function should be carried out before treatment initiation
• LUTS have a significant impact on sexual function1
• Male patients with urinary symptoms should be carefully evaluated for the presence of sexual dysfunction2
• Sexual adverse event rates reported in clinical trials of BPH medications (both alpha blockers and 5ARIs) generally range from 1 to 10%. 3,4 Evidence from the CombAT study suggests the likelihood of sexual AEs with treatment decreases after year 1.5
• The so called ‘nocebo’ effect may magnify the reported incidence of sexual side effects6
• The physician relationship with their patients is fundamental to achieving good results in terms of low incidence of sexual side effects6

Possible explanations for more Gleason 8-10 on second REDUCE biopsy
Dutasteride preferentially suppresses low-grade cancer
• It is possible that 5ARI treatment results in a selective inhibition of low-grade cancers in men whose prostates contain both low- and high-grade cancer1
• Such inhibition could enhance the relative proportion of high to low-grade cancers such that the high-grade tumours would be more likely detected by needle biopsy1,2
• Smaller high-grade tumours that were missed in a previous biopsy might be detected at a later biopsy1

• ESTOS FÁRMACOS SON CAPACES DE PREVENIR LA PROGRESIÓN DE LA ENFERMEDAD CON RESPECTO A LA RETENCIÓN AGUDA DE ORINA Y LA NECESIDAD DE UN TRATAMIENTO QUIRÚRGICO.
