PREFACE

This report presents the results of an occupational survey of theRadiologic career ladder, Air Force Specialty Code (AFSC) 903X0. Authorityfor conducting occupational surveys is found in Air Force Regulation (AFR)35-2. Computer products used in this report are available for use by opera-tions and training officials.

SUMMARY OF RESULTS

1. Survey Coverage: The sample included 655 personnel, or 56 percent of thetotal assigned population. Three-skill level personnel were not included inthe study, as they are students in Phase II training. Major command (MAJCOM)and paygrade groups were adequately represented in the sample.

2. Specialty Jobs: Survey data show there is one cluster of jobs and sevenindependent jobs in the career ladder, distinguished by tasks members performrelated to specific procedures or equipment.

3. Career Ladder Progression: AFSC 903X0 personnel follow an orderly skill-level progression. Five-skill level personnel perform basic radiologic tasks,7-skill level personnel perform a mixture of more technical and supervisorytasks, while 9-skill level and CEM-code members are the managers of the careerladder.

4. AFR 39-1 Specialty Descriptions: The AFR 39-1 Specialty Descriptions forthe Radiologic career ladder provide accurate descriptions ef the jobs andtasks performed at each skill level.

5. Training: Most elements of the Specialty Training Standard (STS) and mostmatched learning objectives of the Plan of Instruction (POI) are supported bysurvey data. Some tasks were not matched to elements of the STS and POI, yetwere performed by sufficient numbers of 903XOs to require review for possibleinclusion in these documtnts.

6. Job Satisfaction: The job satisfaction of AFSC 903X0 personnel is higherthan that of members of related medical AFSCs surveyed in 1991 and higher thanthe previous study. Members performing the various jobs find their jobsinteresting and feel their talents and training are used. Reenlistmentintentions, however, are lower for AFSC 903X0 personnel than those reported bymembers of related AFSCs and lower than those reported in the priorOccupational Survey Report (OSR).

7. Discussion: New procedures and types of equipment have been placed in usewith resulting specialization. Additional Special Experience Identifiers(SEIs) or shredouts may be needed to address the specialized procedures andequipment used. AFSC 903X0 progress typically through the career ladder. TheSTS and POI are well supported, and the AFR 39-1 descriptions are accurate.Job satisfaction has remained high for members of this specialty, whilereenlistment intentions have declined.

OCCUPATIONAL SURVEY REPORTRADIOLOGIC CAREER LADDER

INTRODUCTION

This is a report of an occupational survey of the Radiologic careerladder, AFSC 903X0. The last occupational survey for this career ladder waspublished in July 1985. This survey was requested by the 3790 MTSW/MSC tocollect data that will be used to review career ladder documents in light ofthe new diagnostic procedures now in use, and to help determine if these newprocedures need to be included in formal Air Force training programs.

Background

According to AFR 39-1 Specialty Descriptions for AFSCs 90310/30/50, dated15 Mar 91, 3- and 5-skill level members are responsible for operating equip-ment to produce diagnostic images using radiography, ultrasonography, andcomputerized tomography modalities; instructing and positioning patients toproduce correct exposures; assisting with special imaging procedures; posi-tioning and monitoring patients and equipment for radiotherapy; performingadministrative functions in processing patients and their records; assistingwith Phase II training; and engaging in general radiology activities such asmixing processing solutions, loading and unloading the film holder, and equip-ment preventive maintenance. The 3-skill level is awarded after Phase Icourse completion and the 5-skill level after Phase II course completion.

Seven-skill level members perform special mammography, linear andmultidirectional tomography, scanography, and xeroradiography techniques;assist the radiologist or physician with angiography and lymphangiography; setup and operate accessory equipment; perform radiotherapy administrative func-tions; and enforce health-protective measures.

Nine-skill level members have additional responsibilities for planningand organizing radiology activities; assisting and advising officer-in-chargeon design and development of organizational structure; analyzing workload andestablishing production controls and performance standards for administrativeand technical radiology activities; monitoring all special equipment preven-tive maintenance and procurement; directing subprofessional radiology activi-ties; observing duty performance and actively providing technical andadministrative advice to subordinates; establishing, supervising, and conduct-ing inservice training programs; managing and directing Phase II medicaltraining; and inspecting and evaluating subprofessional radiology activities.

Members enter the career ladder by completing the 14-week Phase I -J3ABR90330-O00 Radiologic Specialist course conducted at Sheppard AFB TX.This course provides instruction on theory of basic electricity; radiographic-techniques; radiographic film and chemical processing; radiographicallyoriented anatomy and physiology; theory and practice of operating fixed andmobile radiologic equipment; routine and special radiographic positioning;theory and practice of special radiographic techniques; and introduction tospecial radiographic procedures and associated equipment. Upon completion,personnel receive the 3-skill level and are sent to a USAF medical facilityfor Phase II training.

Phase II training lasts 38 weeks and provides practical clinical trainingand experience in nursing, orthopedic, and surgical procedures; departmentadministration; film processing and exposure techniques; bedside and surgicalradiographic procedures; night emergency call; and proficiency training inradiologic technology and radiation protection. Upon completion, personnelreceive the 5-skill level. At the present time, the new modalities are taughteither by on-the-job-training (OJT) or through attendance at expensive coursesconducted at civilian institutions.

SURVEY METHODOLOGY

Inventory Development

Data for this survey were collected using USAF Job Inventory (JI)90-903-950 dated November 1990. The inventory developer reviewed pertinentcareer ladder documents, OSRs, and job inventories from the previous OSR, andthen prepared a tentative task list. This preliminary task list was thenrefined and validated through personal interviews with 50 subject-matterexperts at the following USAF hospitals:

The final inventory contained 609 tasks grouped under 14 duty headingsand background questions requesting such information as grade, MAJCOMassigned, duty title, job satisfaction, and equipment used.

Survey Administration

From May 1991 to January 1992, Military Personnel Flights at operationalbases worldwide administered the surveys to AFSC 903X0 personnel selected froma computer-generated mailing list provided by the Armstrong Laboratory, HumanResources Directorate (AL/HRD). Respondents were asked to complete an identi-fication and biographical information section first, go through the bookletand check each task performed in their current job, and go back and use a9-point scale to indicate the relative amount of time they spend performingthe tasks they marked. Time spent ratings ranged from 1 (indicating a verysmall amount of time spent) to 9 (indicating a very large amount of timespent).

The computer calculated the relative percent time spent on all tasks foreach respondent by first totaling ratings on all tasks, dividing the ratingfor each task by this total, and multiplying by 100. The percent time spentratings from all inventories were then combined and used with percent memberperforming values to describe various groups in the career ladder.

Survey Sample

The final sample includes responses from 655 AFSC 903X0 members.Personnel holding the 3-skill level were omitted from the sample because theyare in Phase II training. As shown in Tables 1 and 2, the MAJCOM and paygraderepresentation of the sample is very close to that of the total AFSC 903X0population.

Task Factor Administration

Job descriptions alone do not provide sufficient data for makingdecisions about career ladder documents or training programs. Task factorinformation is needed for a complete analysis of the career ladder. The

survey process provides the needed task factor data by asking selected E-6 andE-7 non-commissioned officers (NCOs) to complete either a training emphasis(TE) or task difficultv (TD) booklet. These booklets were processed sepa-rately from the JIs, and the TE and TD data, where applicable, are consideredwhen analyzing other issues in this study.

Training Emphasis (TE). TE is defined as the amount of structured trainingfirst-enlistment personnel need to perform tasks successfully. Structuredtraining is defined as training provided by resident technical schools, fieldtraining detachments (FTD), mobile training teams (MTT), formal OJT, or anyother organized training method. Forty-six experienced AFSC 903X0 NCOs ratedthe tasks in the inventory on a 10-point scale ranging from 0 (no trainingrequired) to 9 (extremely high amount of training required). The interrateragreement for these 46 rates was acceptable. The average TE rating is 2.70,with a standard deviation of 1.97. Any task with a TE rating cf 4.67 orgreater is considered to have a '.gh TE.

Task Difficulty (TD). TD is defined as an estimate of the length of time theaverage airman takes to learn how to perform a task. Fifty-eight experiencedNCOs rated the difficulty of the inventory tasks on a 9-point scale rangingfrom 1 (easy to learn) to 9 (very difficult to learn). Interrater agreementwas again acceptable. TO ratings are normally adjusted so tasks of averagedifficulty have a value of 5.0, with a standard deviation of 1.0. Thus, anytask with a TD rating of 6.00 or above is considered difficult to learn.

SPECIALTY JOBS(Career Ladder Structure)

The first step in the analysis process is to identify the structure ofthe career ladder in terms of the jobs performed by the respondents. Compre-hensive Occupational Data Analysis Program (CODAP) assists by creating anindividual job description for each respondent based on the tasks performedand relative amount of time spent on the tasks. The CODAP automated job clus-tering program then compares all the individual job descriptions, locates thetwo descriptions with the most similar tasks and time spent ratings, andcombines them to form a composite job description. In successive stages, newmembers are added to this initial group, or new groups are formed based on thesimilarity of tasks and time spent ratings.

The basic group used in the hierarchical clustering process is the Job.When two or more jobs have a substantial degree of similarity in tasks per-formed and time spent on tasks, they are grouped together and identified as acluster. The structure of the career ladder is then defined in terms of jobsand clusters of jobs.

Overview

The mission of the Radiologic career ladder is to provide reliableradiological support in the diagnosis, treatment, and prevention of injuries/diseases affecting the health and welfare of USAF personnel (both active dutyand retired) and their dependents. The Radiology career ladder operates underthe supervision of the USAF Surgeon General and performs a highly visible,integral part of hospital and medical facility services Air Force wide. Theamount and type of service provided by any radiology unit depends on themedical treatment facility it supports. The USAF Surgeon General designatescategories of medical treatment facilities based upon the scope of medicalservices or specialities provided. In turn, the scope of medical servicesprovided is determined by the ability of the professional staff and adequacyof medical facilities. The occupied patient bed rate further delineatesmedical treatment facilities, such that average ranges (as of January 1988)are as follows:

Survey data show the career ladder consists of one general radiology job,five small specialized jobs of advanced modalities, a training job, and asuperintendent job (see Figure 1). These jobs are listed below. The following are descriptions of the jobs identified in the Radiologiccareer ladder. Respondents performing these jobs accounted for 93 percent ofthe survey sample. The remaining 7 percent were performing tasks or series oftasks which did not allow the computer to group them with members of the otherjobs.

I. GENERAL RADIOLOGY CLUSTER (STG55, N=509). This is the core job ofthe career ladder. AFSC 903X0 personnel performing it prepare and positionpatients for radiographic examinations and perform the radiographic examina-tions. Members with this job report performing an average of 156 tasks, moretasks than members of the other jobs perform, and spend most of their timeperforming standard radiographic examinations. The following are typicaltasks performed by members of this cluster:

Most personnel with this job hold the 5-skill level, average almost 7years of total active federal military service (TAFMS), are in paygrade E-4,and are assigned primarily to USAF hospitals and medical facilities.

Within this cluster, five job variations were noted. Two of thevariations involve performing some advanced modality tasks, such as Computer-ized Tomography (CT) and Ultrasound. One involves more OJT functions, anotherincludes both routine X-ray and superintendent tasks, while the last is per-formed by personnel at smaller facilities and involves both routine X-ray andadministrative duties.

IL. ANGIOGRAPHY JOB (STG86, N=8. The Angiography job is quitespecialized and performed by only a few AFSC 903X0 personnel. Members spendtheir duty time performing standard and special radiographic techniques andadvanced diagnostic modalities. As shown by the typical tasks performedlisted below, the job involves performing mainly angiographic procedures.

IV. MAGNETIC RESONANCE IMAGING (MRI) JOB (STG72, N=5). MRI is thenewest modality and the newest job in the career ladder. Those performing thejob spend most of their time performing tasks related to MRI, as shown below:

Members with the job perform an average of only 44 tasks,fewer than members with other jobs in the career ladder. Radiologic Therapypersonnel are somewhat more senior, as they average 9 years TAFMS. This jobis distinguished from the others by the time members spend performing thefollowing tasks:

VIII. SUPERINTENDENT JOB (STG45, N--37. Senior AFSC 903X0 personnel withthe Superintendent job comprise 6 percent of the total sample. The titleSuperintendent is usually associated with 9-skill level and CEM-code person-nel, but members with this job report having this job title. Members withthis job spend 63 percent of their duty time performing supervisory, adminis-trative, and quality control tasks. Most respondents with this job hold the7-skill level, average over 17 years TAFMS, have been in the joc over 15years, and report supervising an average of 10 people. This job is distin-guished from the others by the time members spend performing the followingsupervisory and administrative tasks:

Comparison to Previous Survey

Table 5 compares the jobs identified in the present study to thoseidentified in the 1985 OSR. Four clusters and 10 independent jobs wereidentified in 1984, while 1 cluster and 7 jobs were identified in the presentsurvey. The differences are due to changes in the job inventory and taskclustering procedures now used to help identify jobs performed. Overall, jobsperformed by AFSC 903X0 personnel have not changed since 1984, except for theaddition of the new MRI job.

ANALYSIS OF DUTY AIR FORCE SPECIALTY CODE (DAFSC) GROUPS

DAFSC group analysis allows identification of similarities anddifferences in duty and task performance at the various skill levels. Thisinformation may be used to evaluate how well career ladder documents, such asAFR 39-1 Specialty Descriptions and the STS, reflect what is actually beingdone by career ladder personnel in the field.

Table 6 displays the distribution of DAFSC group members across careerladder jobs, while Table 7 reflects relative time spent by the DAFSC groups oneach duty. These data show most 5-skill level members perform the General

Radiology job, 7-skill level members are first-line supervisors performing amixture of technical and supervisory tasks, and the most senior personnel arethe managers of the career ladder.

There is also a progression as far as types of procedures performed.Five-skill level personnel perform standard radiographic procedures, while7-skill level personnel perform the more difficult radiographic procedures.Nine-skill level and CEM-code personnel perform very few technical tasks,focusing primarily on supervisory, administrative, and managerial tasks.Individual descriptions of the skill levels is presented below.

Skill-Level Descriptions

DAFSC 90350. Personnel with a 5-skill level constitute 73 percent of thesurvey sample. Eighty-three percent perform the General Radiology job, withsmaller percentages performing all but the SuperinteýIent job. Five-skilllevel members perform an average of 128 tasks, most are in paygrades E-4 andE-5, anj they average 67 months TAFMS. Represent, ýi'.? tasks performed by 5-skill level members are displayed in Table 8 and rý-flect their involvement ingeneral radiologic functions.

DAFSC 90370. Seven-skill level mem. rs corstitute 25 percent of the surveysample. They perform an average o, 15E ta.ks and are found in 5 of the 8career ladder jobs (see Table 6). The largest per-- oge work in the GeneralRadiology cluster, wi~h the n,.xt largest percntLage performing the Superin-tendent job. While 50 percen tc1eir total job time is spent on supervisoryand administrative tasks (DL k through E), they are still involved intechnical aspects f 6he caree i ider and have the responsibility of perform-ing the more difficuit procedur -. Represcrtative asks 7-skill level person-nel perform are displayed in Table 9, while tasks which best differentiatebetween the 5- and 7-skill level personnel are displayed in Table 10. Therole oF 7-skill level members as supervisors is clearly shown by the higherpercentages of 7-skill level members performing tasks from Duties A, B, and C.

DAFSC 90399/00. These senior personnel are the managers of the Radiologiccareer ladder. They perform an average of 108 tasks and perform only theSuperintendent job (see Table 6). They report supervising an average of 10people and indicate that 84 percent of their duty time is spent on super-visory, managerial, and administrative duties. Representative tasks 9-skilllevel and CEM-code personnel perform are displayed in Table 11, and taskswhich best differentiate this group from 7-skill levels are displayed in Table12. Figures on the top half of the table show a higher percentage of thesesenior personnel perform supervisory and administrative tasks, and figures onthe bottom show they perform virtually no general radiographic tasks.

Summary

AFSC 903X0 personnel follow an orderly skill-level progression. The5-skill level personnel perform the basic radiologic tasks, 7-skill levelpersonnel have a broader job, with increased supervisory and administrative

responsibilities. The 9-skill level and CEM-personnel are almost exclusivelyinvolved in management responsibilities and perform very few technical tasks.

ANALYSIS OF AFR 39-1 SPECIALTY DESCRIPTION

Survey data were compared to the AFR 39-1 Specialty Descriptions forRadiologic Specialists, Technicians, and Superintendents (all dated 30 April1991). The descriptions for all skill levels are accurate in describing theoverall jobs and tasks performed by members at each skill level in this careerladder. The 5- and 7-skill level descriptions overlap greatly, with the7-skill level description adding the performance of special techniques,administrative functions, and enforcing health protective measures.

TRAINING ANALYSIS

Occupational survey data are a source of information which can be used toassist in the development of relevant training programs for entry-level per-sonnel. Factors used to evaluate entry-level Radiographic training includejobs being performed by first-enlistment personnel, overall distribution offirst-enlistment personnel across career ladder jobs, percent first-job (1-24month TAFMS) and first-enlistment (1-48 months TAFMS) members performingspecific tasks or using specific equipment items, and TE and TD ratings(previously explained in the SURVEY METHODOLOGY section).

First-Enlistment AFSC 903X0 Personnel

There were 191 first-enlistment Radiologic personnel in the sample(3-skill levels members omitted). The distribution across career ladder jobsis illustrated in Table 4 and in Figure 2. Most perform the General Radio-graphy job, with small percentages working in the specialized Angiography, CT,and MRI jobs. Representative tasks first-enlistment AFSC 903X0 personnelperform are listed in Table 13 and deal almost exclusively with generalradiographic procedures. Equipment items used by more than 30 percent offirst-enlistment personnel are listed in Table 14.

TE and TO Data

TE and TO data are secondary factors that can assist technical schoolpersonnel in deciding what tasks should be emphasized in entry-level training.These ratings, based on judgments of senior Radiologic career ladder NCOsworking in the field, were collected to provide training personnel with arank-ordering of tasks considered important for individuals being trained(TE), along with a measure of the difficulty of those tasks (TD). Whencombined with data on the percentages of first-enlistment personnel performing

tasks, comparisons can be made to determine if training adjustments arenecessary. For example, tasks receiving high ratings on both task factors,accompanied by moderate to high percentages of members performing, may warrantformalized resident training. Those tasks receiving high task factor ratings,but low percentages performing, may be more appropriately planned for OJTprograms. Low task factor ratings may highlight tasks best omitted fromtraining for first-enlistment personnel, but this decision must be weighedagainst percentages of personnel performing the tasks, command concerns, andcriticality.

To help in this determination an additional factor, the AutomatedTraining Indicator (ATI) was computed for each task in the inventory. Thisfactor assists school personnel in making training decisions. A computerprogram uses the percent of first-enlistment members performing each task, TEand TD ratings, and the Course Training Decision Table found in Air TrainingCommand Regulation (ATCR) 52-22, Atch 1, to assign an ATI value to each taskin the inventory. ATIs range from I to 18 and suggest what tasks are mostappropriate for training and to what level. The decision table and explana-tion of the ATIs precede the listing of tasks in descending ATI order in theTraining Extract. School personnel will find this table and listing valuablefor making decisions about training documents.

A sample of ta'- dith the highest TE ratings, with accompanying percentfirst-job (1-24 mnnt',ý, TAFMS) and first-enlistment (1-48 months TAFMS) membersperforming, are 1*,ced in Table 15. As expected, tasks with the highest TEratings deal v'ith performing standard radiographic examinations. These tasksare performed by high percentages of criterion group members, but do not havehigh TD.

Most tasks with the highest TO ratings, on the other hand, deal withultrasound, radiation therapy, and advanced modality procedures. These tasks,aloig with percent first-enlistment and 5- and 7-skill level members perform-ing data, are listed in Table 16. All are performed by very low percentagesof criteria group members and most have low TE.

Various listings of tasks, accompanied by TE and TO ratings, arecontained in the TRAINING EXTRACT package and should be reviewed in detail bytechnical school personnel. For a more detailed explanation of TE and TDratings, refer to Task Factor Administration in the SURVEY ADMINISTRATIONsection of this report.

To assist specifically in the evaluation of the AFSC 903X0 STS, technicalschool personnel from Sheppard Training Center matched job inventory tasks toappropriate sections and subsections of the STS. A complete computer listingdisplaying the STS elements, the matched tasks, the percent members performingtasks, TE and TD ratings, and ATI for each task, has been sent to the techni-cal school for their use in further reviews of training documents. STSparagraphs containing general knowledge information, subject-matter-knowledge-only requirements, or basic supervisory responsibilities were not examined.Task knowledge and performance elements of the STS were compared against the

standards set forth in AFR 8-13/ATC Supplement I (Attachment. 1, paragraphA1-3c(4), and ATCR 52-22. Any element with matched tasks performed by 20percent of first-job (1-24 months TAFMS), first-enlistment (i-48 monthsTAFMS), 5-skill level, or 7-skill level members is considered to be supportedand should be part of the STS.

Survey data support a majority of the paragraphs and subparagraphs in theSTS. Specifically, 58 of the 63 paragraphs or subparagraphs matched to surveydata had tasks performed by 20 percent or more of criterion group members.The five areas that were not supported should be reviewed to determine if theyshould be included in the STS. The five unsupported STS elements with surveydata are shown in Table 17.

There were 126 technical tasks performed by 20 percent or more of thecriterion groups and rated high in TE not matched to any paragraph or sub-paragraph of the STS. Examples of these not matched STS items are shown inTable 18. These tasks deal with general radiographic procedures and should bereviewed to determine if they suggest topics that need to be included in theSTS.

POI

Technical school subject-matter experts also matched tasks to theJ3ABR90330-000, Radiologic, POI (dated 16 November 1988). Standards set forthin ATCR 52-22, Attachment 1, dated 17 February 1989, were used to evaluatelearning objectives with tasks matched. Any objective with matched tasksperformed by 30 percent or more of first-job or first-enlistment members isconsidered to be supported by survey data. Learning objectives which do notmeet these criteria should be considered for elimination from the formalcourse, if not justified on some other acceptable basis.

Even though the POI is basically a theory-centered course, 14 of the 103learning objectives are not supported by OSR data. A sar.ple of the unsup-ported learning objectives with survey data are presented in Table 19.

Twenty-six tasks not matched to any block or unit of instruction had atleast 30 percent members of the criterion groups performing. A sample ofthese tasks is presented in Table 20. Several deal with forms and administra-tive procedures, while others deal with standard radiographic procedures.SMEs should carefully review these tasks to determine if they suggest topicsthat should be included in structured training.

An examination of the job satisfaction indicators for various groupsgives career ladder managers a better understanding of some of the factorswhich may impact on job performance of personnel in the career ladder.

Attitude questions covering job interest, perceived use of talents and train-ing, sense of accomplishment from work, and reenlistment intentions wereincluded in the survey booklet. The information from these questions isdiscussed in Tables 21, 22, and 23.

Table 21 compares responses from members of various TAFMS groups in thecurrent study with those from members of related medical AFSCs surveyed in1991. Generally, all indicators are higher for AFSC 903X0 personnel thantheir counterparts in other medical AFSCs. The exception is in reenlistmentintentions. Fewer AFSC 903X0 personnel plan to reenlist than those in theother medical specialties.

Table 22 compares satisfaction indicators for TAFMS groups in the currentsurvey to those of the previous OSR. Respondents to the current studyexpressed higher job interest and perceived use of talents and training.Reenlistment intentions, however, are also lower for the current study.

Satisfaction indicators for members performing the individual specialtyjobs are displayed in Table 23. Members performing all jobs find their jobsinteresting, feel their talents and training are used, but have varyingreenlistment intentions. This may be influenced by specialized experiencegained in the Air Force and employment opportunities in the civilian com-munity.

DISCUSSION

Since the last STS was developed, new procedures and types of equipmenthave been placed in use with resulting specialization. Additional SEIs orshredouts may be needed for the career ladder to address the specializedprocedures and equipment used. AFSC 903X0 progress typically through thecareer ladder with an increase in complexity of procedures performed, and theaddition of supervisory responsibility with experience. Training documentsare well supported and the AFR 39-1 Specialty Descriptions are accurate. Jobsatisfaction has remained high for members of this specialty, while reenlist-ment intentions have declined, no doubt due to employment opportunitiesoutside the Air Force.
